CSA Reporting Form

Please complete and submit a CSA Report as soon as you receive information regarding an alleged Clery-reportable crime that happens within the institutions Clery Geography. Follow the instructions below:

1. Download and open the fillable CSA Reporting Form.
2. Complete all applicable fields with the information available at the time of the report.
3. Save the completed form using the following file name format:
   CSA Report_[Clery Crime]_[Month Year]_[Last Name]
   1. CSA Report_Aggravated Assault_October 2026_Smith
4. Email the completed form to cleryact@lsus.edu
5. Use the following email subject line:
   CSA Report | [Clery Crime] | [Month Year] | [Last Name]
   1. Example: CSA Report | Aggravated Assault | October 2026 | Smith

If the incident is an emergency or requires an immediate law enforcement response, always contact the LSUS Police Department at (318) 455-5497 or call 911 immediately.

Who is Considered a Campus Security Authority (CSA)?

A Campus Security Authority (CSA) is an individual who, because of their role at the institution, is designated under the Clery Act to receive and report information about alleged Clery-reportable crimes. CSAs generally include:

• Campus police and security personnel
• Individuals responsible for campus security, such as those monitoring access to facilities or events
• Officials with significant responsibility for student and campus activities, including many administrators, deans, directors, student affairs staff, athletic administrators, club advisors, and others with substantial student interaction
• Officials specifically designated to receive crime reports by the institution

Generally, faculty members without significant student responsibility, clerical staff, and professional counselors acting within the scope of their confidential counseling role are not CSAs.

Campus Security Authority (CSA) Reporting Responsibilities

All members of the campus community are encouraged to report crimes, suspicious activity, and safety concerns to LSUS Police, regardless of whether they are designated as a CSA.

Campus Security Authorities (CSAs) are responsible for promptly submitting a CSA Report Form or reporting directly to LSUS Police when they receive information that appears to meet the criteria of a Clery-reportable crime, even if the information is limited, incomplete, or unverified. Additional information can be provided if it becomes available. This helps the university comply with federal requirements and determine whether a campus safety notification is needed. If you are unsure whether an incident is reportable, submit a report.

Individuals who are also designated as Mandated Reporters must also report incidents involving sexual misconduct, sexual assault, dating violence, domestic violence, stalking, or other Title IX-related conduct to the Title IX Office using the Sexual Misconduct Reporting Form in accordance with LSU Permanent Memorandum 73, which can be found on the LSU Permanent Memoranda web page.

CSAs must also complete an Annual CSA Reporting Confirmation, which is sent via email through software called Clery Edge. This confirmation helps ensure that all Clery-reportable crimes known to a CSA have been reported through the proper process. CSAs are asked to confirm whether they received any reports of Clery-reportable crimes during the year and that those incidents were submitted, if applicable.

Campus Security Authority (CSA) Training

LSUS Campus Security Authorities (CSAs) identified by the University must complete annual Campus Security Authority Training. CSAs will receive this training through software called Clery Edge, which emails a link to complete the training and tracks completion status.
